Welcome to our practice! We appreciate the confidence and trust placed in us by allowing us to care for your cat.
Before your first appointment, please print and fill out the new client information sheet. Also, please bring any relevent vaccine records with you. If your cat has any chronic medical issues, we will need your previous veterinarian(s) to forward the complete records to us. Please ask specifically for doctor's notes, labwork, and x-rays.
If you cannot keep your appointment, please call us as soon as possible at 513-779-3300 so that we may offer the appointment to another client.
